To get started, please fill out and submit the online Consultation Request Form. A Pine Pharmacy pharmacist, who specialize in teaching people the importance of adequate nutrition and vitamin supplementation will review your consultation request and will send you an email with a link to a questionnaire. The pharmacist at Pine Pharmacy will contact you upon receipt of the completed questionnaire to finalize the execution of the nutritional assessment.

The fee for this nutritional assessment is $100.00 USD.

Saliva testing is a very reliable way to measure free, “bioavailable” hormone activity—hormones actually doing their job at the cellular level. Saliva collection is easy and can be done in the convenience of your own home with results sent directly to a Pine Pharmacy pharmacist to be evaluated and discussed with your physician. This test provides a rational baseline of current hormone activity when developing a bio-identical hormone replacement therapy plan for individual men and women.
